There are of course, a whole host of things that you will need to provide us in order to start your payroll processing with Solutions. What you need varies by how you are currently setup. If you are a new business just starting your first payroll there will be far less information to provide. This list will give you an idea of what information your should start gathering. Depending upon your specific needs, there may be additional information. Our implementation team will assist you through the process.

- Client Services Agreement
- New Client Setup Information Form
- Bank Account Signature Form
- Direct Deposit Services Agreement
- Employee Direct Deposit Enrollment Form
- Tax Information Authorization (IRS Form 8821)
- Tax Deposit Authorization (IRS Form 8655)
- Tax Forms Filing Authorization (IRS Form 2848)
- A voided check for each bank account to be used to debit funds for payroll, taxes and billing
- Current quarters employee payroll information by pay period for each employee
- Each employees quarter to date and year to date payroll information
- Company current quarter and year to date information on Gross Pay, Social Security Payments, Medicare Payments, Federal and State Tax Payments, Deductions and New Pay.
- Current quarter's liabilities and deposits per pay period for Federal Tax deposits with dates of deposits.
- Current quarter and year to date deposit and liability information for SUI
- Copies of official documents verifying Business Name, Address and Tax ID Number.
- Worker's compensation Codes and Breakdown by employee.
